ANALISIS KELENGKAPAN BERKAS REKAM MEDIS BERDASARKAN INFORMED CONSENT PADA PASIEN JANTUNG RUMAH SAKIT SANTA ELISABETH MEDAN TAHUN 2024

Abstrak

Rumah sakit adalah institusi pelayanan kesehatan yang menyelenggarakan pelayanan kesehatan secara paripurna. Rekam medis merupakan dokumen penting bagi setiap instalasi layanan, kesehatan yang berupa cacatan pasien, pemeriksaan, pengobatan, tindakan dan pelayanan lain yang diberikan kepada pasien. Rekam medis yang lengkap merupakan citra mutu dari sebuah rumah sakit. Tujuan Penelitian ini adalah untuk menganalisis kelengkapan berkas rekam medis berdasarkan informed consent pada pasien jantung di Rumah Sakit Santa Elisabeth Medan pada tahun 2023 . Jenis penelitian yang digunakan adalah penelitian deskriptif retrospektif. Sampel pada penelitian ini adalah sebanyak 84 rekam medis. Pengambilan sampel dilakukan dengan metode Systematic Random Sampling. Instrumen yang digunakan adalah lembar checlist. Hasil penelitian diperoleh persentase kelengkapan untuk informent consent nama pasien sebesar 84 (100%) terisi lengkap, tindakan persetujuan pengobatan 70 (83,3%) yang terisi lengkap, tanggal dan tanda tangan pasien/keluarga 54 berkas (64,28%) terisi lengkap dan tanda tangan administrasi dokter sebanyak 70 berkas (83,3%) yang terisi lengkap. Penyebab ketidaklengkapan pengisian berkas informent consent ini diakibatkan kurang teliti petugas dalam menuliskan formulir pasien kesibukan dokter/perawat . Diharapkan bagi petugas rekam medis, perlu memiliki kesadaran dan kedisiplinan dalam melengkapi rekam medis dengan cara kerja sama yang baik antara perawat, dokter yang bersangkutan.

Complete medical records are an image of the quality of a hospital. The aim of this research is to analyze the completeness of medical record files based on informed consent for heart patients at Santa Elisabeth Hospital Medan in 2023. The type of research used is retrospective descriptive research. The sample in this study was 84 medical records. Sampling was carried out using the Systematic Random Sampling method. The instrument used is a checklist sheet. The research results showed that the percentage of completeness for informed consent for patient names was 84 (100%) filled in completely, treatment consent measures were 70 (83.3%) were filled in completely, dates and signatures of patients/families were 54 files (64.28%) were filled in completely. and the doctor's administrative signature of 70 files (83.3%) were filled in completely. The reason for the incompleteness of filling out the informed consent file is due to the staff not being careful in writing the patient form for the doctor/nurse's busy schedule. It is hoped that medical record officers need to have awareness and discipline in completing medical records by means of good cooperation between the nurses and doctors concerned.
